​If you wish to have a copy of your records sent to you or to another provider or facility, you may submit a written request to this office before the Effective Date. After the Effective Date you will need to submit a request form with the Records Custodian. The release form can be found on their website:

There are a few ways you can request your medical record until 12/19/25:
1) You can fill out a medical record request form at your new office and have them fax it to 303-797-8448.
2) You can log in to your patient portal and send a request with the name of the practice along with their fax number.
3) You can click here to access a copy of our medical record request form. You can then fax the form to 303-797-8448.

I will be closing Generations effective December 19, 2025 and I will be relocating to a new practice in March 2026.
It has been a privilege to care for you, and I sincerely appreciate your trust and confidence over the years. I will be taking a sabbatical until March 4, 2026 but look forward to continuing to care for you upon my return. I am committed to ensuring a smooth transition to my new practice:

Our electronic medical record will continue to have an active patient portal until my transition to the new office. In my absence; between December 19, 2025 through March 1, 2026, Dr. Oster, Dr. Kohls, Jamie Luck NP as well as other midlevel providers will be happy to care for you.
If you are a BioTe patient my new office will also be offering this service. The price will increase; as the price is set by the new office and I will be an employee of this office. The new price will be $365.00. You should have received a paper at your last pelleting appointment documenting the dose you are receiving. Please bring this to your appointment.
